Procedures

GUIDE IN THE PROCESSING OF BUSINESS PERMITS (NEW/RENEWAL)
  1. Obtain forms at the City Treasurer's Office-Business Taxes & Fees Division and fill-out & completely all blanks in the forms.

    • Application form for new business
    • Sworn Statement for Renewal of Business Permit

  2. Proceed to the City Treasurer Office-Realty Division for Tax Clearance (Attach copy of Tax Declaration or previous official receipt).

  3. Bring your filled-out forms with the barangay clearance and Tax Clearance to the City Planning & Development Coordinator for approval signature of Zoning (For new business only).

  4. Market Clearance (For Market Stall Holder Only)

  5. Go back to the Business Taxes & Fees Division for assessment of your business taxes & attach the following papers:

    • Application Set for New Business w/

      1. Articles of Incorporation/Partnership (For new business)

    • Sworn Statement for Renewal of Business Permit w/

      1. Mayor's Permit of previous year or quarter
      2. Latest Income Statement/Income Tax Return/Quarterly report duly received by the BIR.

    For Assessment :

    • Arden Quinaud - Cash Clerk I
    • Naomi Seno - Clerk III
    • Aurelia Ceniza - LTOO I
    • Djalma Regudo - LRCO I
    • Engelberto Ouano - LRCO I
    • Charles Evan Dabon - Program

    For Approval :

        Mr. Hubert Ortega
        LRCO II
        (Actg.Chief Bus. Taxes & Fees Div.)

  6. Secure Community Tax Certificate (CEDULA)

    • For Individual/Sole Proprietor - CTC (Cedula Section)
    • Corporation/Partnership - Romeo Go - LTOO III

  7. Proceed to the Asst. Treasurer, Mr. CIPRIANO L. JAMORA for final evaluation & approval of payment.

  8. Pay business taxes based on the assessment either to the following collectors

    • Merve Z. Flores - RCC II - General Receipt Section
    • Ana S. Quirante - LRCO I - General Receipt Section
    • Mariter B. Rosales - Cash Clerk I - General Receipt Section
    • Divina Ybañez - Program - General Receipt Section

  9. Proceed to the City Administrator's Office for preparation of the Mayor's Permit bringing with you the Attach Set/ Renewal Set Form duly assessed and paid and attach the following:

    • SSS Clearance / Pag-ibig Clearance / Philhealth Clearance
    • DTI Cert. of Registration - for Sole Proprietorship
    • SEC Reg. for Corporation/Partnership

  10. Go to the City Health Department for issuance of Sanitary Permit

  11. Have your Fire Certificate signed by the City Marshall at the City Fire Dept.

  12. Go back to the City Administrator for final approval and signature on the Mayor's Permit.


Note: 2004 Mandaue City business plates will be issued at cost of P515.00 for display during tax monitoring and inspection.
