and fill-out & completely
all blanks in the forms.
- Application form
for new business
- Sworn Statement
for Renewal of Business
Permit
- Proceed to the City
Treasurer Office-Realty
Division for Tax Clearance
(Attach copy of Tax
Declaration or previous
official receipt).
- Bring your filled-out
forms with the barangay
clearance and Tax Clearance
to the City Planning
& Development Coordinator
for approval signature
of Zoning (For new business
only).
- Market Clearance (For
Market Stall Holder
Only)
- Go back to the Business
Taxes & Fees Division
for assessment of your
business taxes &
attach the following
papers:
- Application Set
for New Business w/
- Articles of
Incorporation/Partnership
(For new business)
- Sworn Statement
for Renewal of Business
Permit w/
- Mayor's Permit
of previous year
or quarter
- Latest Income
Statement/Income
Tax Return/Quarterly
report duly received
by the BIR.

Department for issuance
of Sanitary Permit
- Have your Fire Certificate
signed by the City Marshall
at the City Fire Dept.
- Go back to the City
Administrator for final
approval and signature
on the Mayor's Permit.
Note: 2004 Mandaue City
business plates will be
issued at cost of P515.00
for display during tax monitoring
and inspection. |
